How do I get started?
When a parent wishes to enroll in therapy, there are two ways to get started: One way is to complete the intake form online here. The second method is to call the main line to schedule a time to talk with the office manager to discuss the issues of concern. This is called a phone intake. Once the phone intake is completed, the family is scheduled an appointment for an intake interview with a clinician. This interview can take between 60 and 90 minutes. It is recommended that families to arrive early to begin completing paperwork that can be discussed further during the intake interview. Both the parent(s) and child should attend the intake interview.
Does anyone at For A Child, LLC work with adults?
For A Child, LLC Associates, Dr. Jamila Brown and Dr. Trista Huckleberry also work with adult clientele. Any adults calling in to the group will be referred to one of them.
How much does this cost?
The fees for services vary based on the level of insurance coverage. Some policies will cover 100% of the costs (less the patient copay), others pay at a lower percentage rate. Prior to the intake interview, an insurance eligibility determination will be made by the office manager. This will allow the family to learn the amount of coverage and actual rate per session at the time of the intake. The patient responsibility amount must be paid at the time of each service.
What insurance plans are accepted?
Dr. Jones is a preferred network provider with the following companies: Aetna, CorpHealth, PacifiCare, Premera Blue Cross, Regence Blue Shield, Magellan Behavioral Health, and Uniform Medical Plan. Other insurances are accepted and will be billed directly; however, the patient responsibility amount may be higher since she is not a network member.
Dr. Brown is a preferred network provider with the following companies: Premera Blue Cross, Uniform Medical Plan, KPS, and Regence Blue Shield. Other insurances are accepted and will be billed directly; however the patient responsibility amount may be higher as part of the out-of-network benefit.
Dr. Huckleberry is a preferred provider for Aetna, Cigna, TriWest, and Regence. She can bill as an out-of-network provider for all other insurance companies. Other insurance companies will also be billed directly; however, the patient responsibility amount may be higher as part of the out-of-network benefit.
Dr. Hukic is in the process of credentialing with Premera Blue Cross, First Choice Health, and Regence Blue Shield. She will bill as an out-of-network provider for all other insurance companies. The insurance companies will be billed directly, but the patient responsibility amount may be higher as part of the out-of-network benefit.
Ms. Dunn is in the process of credentialing with insurance companies. She will bill as an out-of-network provider for all other insurance companies. The insurance companies will be billed directly, but the patient responsibility amount may be higher as part of the out-of-network benefit.

Typically families are served for as few as five months and as long as twelve months. The amount of time varies based on the complexity of the issues of concern.
What if I have to cancel my appointment?
Cancellations require 24 hour notice. This policy is to open appointment times to other families in need. As long as 24 hour notice is provided, there is no charge to the patient. The first time there is a late cancellation, there is a charge of $25. For each additional occurrence of late cancellation, you will be charged the full fee for the session. This charge is NOT covered by insurance.
