Psychological Assessment comes in many forms and For A Child, LLC receives referrals for a variety of needs. For example, some families self refer to have their child evaluated for an admissions application to private school. Other families seek evaluations to assist in developing behavioral interventions for school. Third party referrals may come from social workers or school personnel who are developing intervention plans to support children on their caseload. Each assessment is individually tailored to the age and needs of the child or adolescent. All charges for assessment are bundled, not charged at an hourly rate. The Assessment descriptions below shows some of the typical requests for assessment along with the services included in each category.
All assessments include a 50 minute feedback session. At the feedback session, the results of the evaluation are shared verbally and in writing. The family is invited to ask questions to obtain a clear understanding of the findings. For an additional fee, Dr. Jones also offers a special service for children to receive assessment results in a child oriented format. These feedback reports are written in the form of a children’s story or a letter. This feedback is provided in addition to the formal report.
Cognitive/ Intellectual Assessment
This assessment is typically requested by parents who are applying to have their child admitted into a private school or applying to a gifted education program. For some schools, the admission process requires the parent to seek an assessment to provide an indicator of their child’s cognitive ability. This is also the type of assessment that is needed for eligibility for APP and spectrum programs (Kindergarten and 1st grade). In this case, children are screened through their school. Those who are close to qualifying (but are not deemed eligible) have the option to appeal the decision. The appeal application typically includes the results from a cognitive assessment.
This assessment includes: interview, administration of cognitive test (e.g. WISC, WPPSI, SB-V, or WJ), evaluation of developmental history, report with full interpretation, recommendations, and an addendum of scores.
Psychoeducational Assessment
This assessment is requested by parents who seek to learn their child’s learning capabilities and academic strengths, as well as cognitive and academic weaknesses. It is best for the determination of learning disabilities, cognitive disabilities, and cognitive and academic giftedness. This is the type of assessment that is required for an appeal for Advanced Placement Programs (APP) for children in 2nd grade or higher.
This assessment includes the following: interview with parents and child, review of academic records (if provided), administration of cognitive, academic, and behavioral assessment measures, and a report with interpretation, recommendations and an addendum of scores.
Psychological Assessment
This assessment is often requested by parents, school psychologists and counselors, family court officials, social workers, and pediatricians. These evaluations are used to determine a child’s current level of emotional and behavioral functioning. Typically the evaluation is sought to determine whether a child or adolescent meets criteria for a particular diagnosis and whether further intervention is necessary.
This assessment includes the following: clinical interview with parents/caregivers and the child, a review of previous psychological or psychiatric evaluations, administration and interpretation of personality and socio-emotional assessment measures. The final product includes a report with interpretation, diagnosis, and intervention recommendations.
Full Battery Assessment
The full battery assessment is ideal when there is a question of learning issues as well as emotional issues. The assessment includes all of the components from the psychoeducational assessment and the psychological assessment.
Attention-Deficit/Hyperactivity Disorder Screening
This screening is to determine whether a child or adolescent is likely to meet criteria for a diagnosis of AD/HD. For this assessment, behavior rating scales, sections of cognitive and neuropsychological tests, continuous performance tests, and clinical interviews are completed. Additionally, in some cases, a classroom observation is made. Families receive a report with diagnosis and recommendations for intervention.
Child Feedback Report
This report is recommended for any type of assessment that was previously described. This report is written specifically to the child and provides them feedback about the results of the assessment in language that is developmentally appropriate. For younger children, this report comes in the form of a storybook. For older children, it is written as a letter, song, or poem.

Applications for private schools are due in February. Make your appointment early for these evaluations. Dr. Jones typically completes the evaluation, report, and feedback session within 14 days. Thus, these appointments must be scheduled at least 3 weeks before the deadline.
Appeals for Advanced Placement Programs (APP) are on a very short timeline. From the time that the letter is sent, most families have two weeks to respond (this includes having the assessment report in hand). If your child has previously received a score that was close to qualifying (but did not qualify), you may wish to schedule your appeal appointment well in advance of the letter. Dr. Jones will only accept appointments that she can meet the deadline.
